Before You Submit: A Practical Clean-Claim Checklist

A claim can move quickly through a clearinghouse and still encounter a preventable problem. A deliberate pre-submission routine helps billing teams identify missing or inconsistent information before the claim reaches a payer.

Begin with the information that anchors the claim

Confirm the patient’s name, date of birth, insurance identifiers, and billing address against the current registration record. A small registration mismatch can create avoidable follow-up.

Check the rendering and billing provider identifiers, service location, and payer selection. Be especially careful when clinicians work across multiple locations or billing arrangements.

Review the dates of service, procedure and diagnosis coding, units, modifiers, and documentation relationships. Coding decisions should be supported by the record and applicable coding guidance; this checklist is not a substitute for a qualified coding review.

Verify coverage and authorization details

Confirm coverage for the relevant service date rather than relying only on an older eligibility check. Investigate inconsistent primary or secondary coverage details before transmitting the claim.

Where authorization or referral requirements apply, confirm the correct payer workflow, reference numbers, approved services, and date range. Authorization does not itself guarantee payment.

Make the submission traceable

Record the original submission date, destination payer, clearinghouse response and payer acknowledgement where available. Different response stages answer different questions: accepted for electronic processing does not mean adjudicated or paid.

Use a defined owner and follow-up queue for a rejected transaction. Fixing the root field error before resubmission is more reliable than repeated transmissions without review.

A simple operational check

Choose a small sample of recent submissions and ask: Was eligibility current? Did each claim have a traceable acknowledgement? Who owned rejected transactions? What types of corrections repeated? Document findings and adjust the intake checklist where recurring patterns appear.
