Prior authorization work can involve clinical documentation, payer requirements, scheduling coordination, and multiple status updates. A shared tracking process helps teams know what is needed, what has been sent, and what still needs a decision.
Confirm whether authorization applies
Check the actual payer, plan, service, provider, location, and proposed date of service. Requirements may differ by payer and benefit design, and not every service requires prior authorization.
Document the source and date of the requirement check. Do not assume that a prior authorization approval resolves eligibility, coverage, or payment questions.
Assemble a complete request
Coordinate with the authorized clinical team to obtain documentation that supports the requested service. Confirm procedure details, relevant identifiers, clinical justification, and payer-specific submission instructions.
Keep protected patient information in approved clinical or payer channels. A public website enquiry form is not an appropriate route for authorization packets.
Track decisions and next actions
Record the request date, submission channel, payer reference number, requested and approved services, validity period, status, responsible owner, and next follow-up date.
If the payer requests more information, record exactly what is needed and which team is responsible. A denial may require a distinct review or appeal pathway under applicable rules.
Keep policy changes in perspective
CMS has adopted prior authorization process requirements for certain impacted payers, but scope, timing, and exceptions matter. The published CMS guidance explains that some decision-timing requirements differ by program and do not apply to every payer or drug request.
Before quoting a deadline to a provider or patient, verify the specific payer’s current requirements and the rules that apply to that request.
