A/R Aging: Turning an Outstanding Balance Report Into Action

A focused method for turning aging buckets into workable priorities, ownership, and measurable follow-up outcomes.

An accounts receivable aging report is a snapshot, not a recovery strategy. The value comes from combining age with payer status, workability, contractual deadlines, and the next clear action for each outstanding balance.

Separate age from urgency

Traditional aging buckets can show how long an amount has been outstanding, but an older balance is not always the most actionable. First distinguish payer-responsibility, patient-responsibility, credit, and adjustment items.

Identify claims approaching timely filing, appeal, or other payer-specific deadlines. Some newer accounts may need immediate attention because a deadline is closer than the balance age suggests.

Build a usable follow-up view

For each account, capture claim number, payer, balance, age, latest response, most recent follow-up date, responsible team member, next action, and review date.

Group work by reason: no claim on file, pended review, missing documentation, recoupment, underpayment question, denial, or unapplied payment. This prevents a single generic “pending” label from masking different problems.

Use consistent follow-up documentation

A useful note records the date, contact or channel, what the payer confirmed, reference number if provided, next requirement, and a deadline for follow-up. Notes should support continuity when work changes hands.

Claim status inquiries can be made through payer portals and electronic channels where supported. CMS documents Medicare 276/277 transactions for request-and-response claim status workflows.

Measure movement, not just activity

Review how balances shift between aging categories, how many claims receive a documented next action, and whether preventable categories recur. A high volume of calls alone does not demonstrate effective resolution.

Return patterns to registration, coding, authorization, or posting teams when the underlying cause sits upstream.
