Eligibility verification is more useful when it creates a clear handoff to scheduling, registration, and billing. A coverage response is the beginning of the conversation, not a guarantee that every billed service will be covered.
Verify the right patient and the right date
Start with the identifying information required for the payer enquiry and check effective dates for the actual scheduled service. A card on file may no longer reflect the current plan or benefit year.
Determine whether the queried payer is primary, secondary, or another coverage source. Investigate mismatched plan information rather than leaving the billing team to discover it after service.
Look beyond an active-coverage flag
Review whether the service, provider, facility, and network arrangement have specific benefit conditions, when available from the payer. Note deductible, copayment, coinsurance, and applicable limitations only to the extent the response supports them.
Identify services that may require prior authorization, referrals, or additional review. Coverage, authorization, and eventual payment are separate decisions.
Create an exception workflow
Flag incomplete, contradictory, or unavailable verification responses. Assign an owner for direct payer follow-up where necessary and communicate unresolved questions to the appropriate front-office team before the visit.
Document when and through which channel the information was checked. Keep access to eligibility and patient information within approved systems and authorized staff workflows.
Use the response without overpromising
CMS describes the 270 request and 271 response for Medicare real-time eligibility checks through HETS. The returned eligibility information supports claim preparation and beneficiary-liability estimation, but claims remain subject to the program’s rules and final processing.
